Frequently Asked Questions for Parents:

  • Providing contact information and a document requesting student grades, as well as completing an online survey about your child’s thoughts and daily behaviors.

  • We will provide a $20 gift card upon completion of the online survey.

  • Although we recommend completing the survey in one sitting, there is an option provided to save and return later should it be needed.

  • The study will collect contact information, demographic information, and answers regarding your child. All information is secure and seen only by the research team. No answers will be linked to you or your child’s name.

  • You will be completing online consent procedures, as well as an online survey regarding your thoughts and feelings

  • We will provide a $20 gift card upon completion of the online surveys

  • You can fill the survey out on your own, your parent does not have to be with you.

  • Once you submit your survey, the only people who will be able to access your answers are the research team.

  • If you are not comfortable answering a specific question, you may type “N/A” in the box/ select “N/A” from the dropdown menu.
